Abstract

본 증례는 전북대학교 치과대학 병원에 구개부의 외방성 종괴를 주소로 내원한 30세 남자
환자로서 임상검사, 방사선학적 검사 및 병리 조직학적검사를 통하여 유선낭종암으로 확진
되었으며 다음과 같은 특징적인 소견이 관찰되었다.
1. 임상적으로 구개적인 부위에 무통성의 외방성 종괴가 관찰되었다.
2. 방사선사진상에서 좌측 상악동, 비강, 측두하와, 경구개, 익상판을 포함한 익돌구개와부
가 침범되었으며 CT상에서 증강된 연조직괴가 관찰되었다.
3. 병리조직학적으로 관상형과 충실성 형의 선구조가 나타났으며 종양조직이 신경조직내
로 침윤되는 소견을 보였다.
4. 외과적 절제, 방사선치료 및 화학요법을 병용한 치료 2년후 신경 주위전파로 인한 안와
부로의 전이가 관찰되었다.
5. 재발과 타 부위로의 전이를 조기에 진단하기 위하여 향후 최소 5년 이상의 장기적인
재검진이 요구된다.

This is a report of adenoid cystic carcinoma occurred in the palate in 30-year-old
patient with a complaint of exophytic mass.
The authors diagnosed it as adenoid cystic carcinoma by the 치inical examination,
radiographic findings and histopathological findings.
The obtained results are as follows:
1. In clinical examination, asymptomatic exophytic mass of palate was observed.
2. In radiographic findings sort tissue mass infiltrated the left maxillary sinus nasal
ovity, infraorbital fossa hard palate, pterygopalatine fossa and pterygoid plate, and
enhanced soft tissue mass was also observed in CT.
3. In histopathological findings, tubular and solid patterns of glandular structures were
observed and the infiltration of tumor cells into the nerve fibers was also observed.
4. Two years after radical surgery, radiation therapy and chemotherapy, the perineural
spread to orbital area was observed.
5. Much longer follow-up than 5 years is needed for early diagnosis of recurrence and
distant metastasis
